Change of Payment Method to auto debit from Credit Card

1. If you would like to change or apply for auto payment facility through major Visa / Master credit card, you are required to:

• Complete and sign the Credit Card Debit Authorization form. Both the card member and policy owner signatures are required. We accept third party credit card for premium payment if the card member is an immediate family member to the policy owner.
• Initial on the second page of the Credit Card Debit Authorization form.
• Provide us with a copy (both sides) of your Visa / Master card for verification.

Please ensure that your signature is executed in the same manner as that in the original of your application form.
